This document was found in the papers of Howard Benjamin Samuel (born Old Warden 1887) who worked at the Powage Press, Aspley Guise, prior to 1914. He then served in the Royal Army Medical Corps and was awarded the British War and Victory medals, bar mounted and inscribed to 122522 Pte H Samuel, R.A.M.C.
